plan turns the analyzed request into the contract used by downstream skills.
Output
The issue records the problem, desired behavior, scope, exclusions, implementation decisions, acceptance criteria, and verification requirements. After creation, the agent reads the issue back from GitHub and returns its URL.
Boundary
The stage is incomplete until the provider read-back confirms the issue. It does not implement the issue.
